Privacy Policy Outline
What personal information do we collect?
The information we collect is only that which is required for us to provide you with the service you have requested. For our online services this may include: your name, address, email address, work title, employment history, education, telephone and fax numbers, and (when purchasing a product) your credit card details. For the services described under Psychology Services, information will be acquired through the normal counselling and therapy and assessment process. Some of this information may be highly sensitive and will form part of the psychologist's notes contained in your personal file which is held under highly secure conditions. We will take all reasonable steps to ensure that the personal information we collect is accurate, complete, and up-to-date.
How do we protect your personal information?
We will take all reasonable steps to protect the personal information we hold from misuse and loss and from unauthorised access, modification or disclosure. In general, you will always be advised before hand if your information is to be disclosed to third parties and we will seek your written and express permission where appropriate. Where you have authorised an online assessment at the request of an employer or prospective employer then your personal information and your assessment will be forwarded to the authorised representative of the employer and these parties are required to treat your information as confidential. However, and despite our precautions, we cannot in these circumstances guarantee that unauthorised persons will not obtain access to your personally identifiable information. As health care providers we may disclose health information about an individual under certain conditions without consent including where we reasonably believe that such disclosure is necessary to lessen or prevent serious and imminent threat to an individual's life, health or safety or a serious threat to public health or public safety. While we take all reasonable precautions to protect data transmitted via the Internet, you should be aware that such transmissions cannot be guaranteed to be 100% secure.
Access to your personal information
If we hold personal information about you, we will provide access to that information upon request from you (a reasonable administration fee may be charged). There are exceptions to access including where to do so would pose a serious threat to the life or health of any individual, would unreasonably impact upon the privacy of other individuals, or where providing the access would be unlawful. If the information you access is not accurate, complete, and up-to-date, we will take reasonable steps to correct the information.
